Logos come in four file types: ai, eps, svg and psd (all transparent backs with graphic on a separate layer from the text (full logo in psd and graphic in the ai, eps and svg vector forms). You will view three concepts based on your answers to the questions on the form you fill out- please feel free to also include any extra info that my help in creating a look that is "you".
